PRN, Care Manager/Care Advocate (W2) 
Average of 5-10hours per week
Monday- Friday 9AM- 5PM

  • Work with a variety of adult/aging clients needing an unbiased professional to assist them and their decision-makers with guidance related to healthcare, aging, and ongoing care planning.

  • Develop care plans and specific client-stated goals towards improving their quality of life.  

  • Serve as liaison between ancillary services and direct the application of these services for the benefit of the client in accordance with the agreed-upon working care plan.  

  • Coordinate care in a variety of situations in a collaborative manner with a variety of healthcare professionals and settings. 

  • Create and apply your knowledge and expertise on a one-on-one client basis.

 

 

The ideal candidate practices ethical considerations in all client interactions and has the ability to think universally instead of a simple task-focused mindset.  

Required Education & Experience: 

Masters degree preferred. Gerontology, social work, nursing, psychology, applied sciences, or related fields. 

 

Minimum 2-3 years of direct care and/or care management experience, including experience in the geriatric field. 

 

Demonstrated clinical knowledge of aging, dementia, movement disorders, and various chronic diseases. 

 

Experience working with complicated family systems and family dynamics is essential.
